3) it's a fairly well known alternate cover Ditko did at the time, Marvel decided to have it redone by Kirby. Nothing new. A google search would've found you the answer.

It was drawn by Steve Ditko around the same time Jack kirby's cover was drawn. It dates back to 1962. It would appear in fanzines or be shown inside some Marvel comics, but it was never used as an actual cover until Amazing Spider-Man #700. It was used as an incentive cover and retailers had to order a lot of the regular version to get the Ditko variant.
